However, translation into practice in a local context can be problematic. In Hong Kong, there is an emerging practice around interdisciplinary professional teams with social workers acting as centralized coordinators. Semi-structured in-depth interviews with 52 service users and family carers, along with two focus groups with 20 professionals, reveal the power of a relational analysis for understanding social work practice within case management teams for home-based community care services for frail older adults. |
